Inspect:
- swelling over scrotum
- no scar, no skin change
- no cough impulse
Palpate:
- non-tender
- normal temperature
- smooth
- firm
- can get above
- no cough impulse
- normal testicle
- site: above testicle, behind cord
- fluctuant
- transilluminant (only cyst transilluminates, scrotum doesn't. differentiate from hydrocele by reduced transilluminance)
Mx:
- USG scrotum
- conservative management
- may enucleate if symptomatic (subfertility)
