Hip examination

Inspect:
  • posture
  • lordosis/scoliosis
  • pelvic tilting
  • leg length disreptency
  • muscle wasting - gluteus, quadriceps, small foot muscles
  • high arched foot
  • surgical scars - posterior and lateral
  • gait - ?short limb gait, antalgic gait
  • Trendelenburg test
Palpate:
  • leg length
    • apparent - from xiphisternus to med malleolus (eg. adduction contractures,)
    • true - ASIS to med malleolus (determine tibial or femoral shortening; above or below trochanter)
  • tenderness - ASIS, greater trochanter, head of femur (just lat to femoral art), iscial tuberosity (hamstring attachment)
Movement:
  • Thomas test - fixed flexion deformity
  • active and passive ROM (square pelvis at ASIS)
    • flexion: 120
    • extension: 15
    • abduction: 40
    • adduction: 25
    • internal rotation: 45
    • external rotation: 40
  • power and reflex
  • sensation
