Hip examination
Inspect:
- posture
- lordosis/scoliosis
- pelvic tilting
- leg length disreptency
- muscle wasting - gluteus, quadriceps, small foot muscles
- high arched foot
- surgical scars - posterior and lateral
- gait - ?short limb gait, antalgic gait
- Trendelenburg test
Palpate:
- leg length
- apparent - from xiphisternus to med malleolus (eg. adduction contractures,)
- true - ASIS to med malleolus (determine tibial or femoral shortening; above or below trochanter)
- tenderness - ASIS, greater trochanter, head of femur (just lat to femoral art), iscial tuberosity (hamstring attachment)
Movement:
- Thomas test - fixed flexion deformity
- active and passive ROM (square pelvis at ASIS)
- flexion: 120
- extension: 15
- abduction: 40
- adduction: 25
- internal rotation: 45
- external rotation: 40
- power and reflex
- sensation
